Individualised Care

Personalised care plans are developed with the Service User to ensure the individual’s needs are met promoting independence and personal development at the heart of a quality, flexible plan.

The Manager, Patrycja Dzierzanowska, is registered with the Care Quality Commission as the Manager of Five Ways. The Manager can be contacted at the Home and has the day to day responsibility for the running of the Home.

Pat is supported by the group structure which includes learning disability specialists.  Also employed directly at Five-Ways is one senior member of staff and care staff who have an on-going program of training and development which enables them to plan and deliver the appropriate care to meet the individual needs of our Service Users.

Pat, as manager, is also responsible for financial matters at the home with the support of the Contemplation Care financial team at our head office which is based in Waterlooville.


Activities and the promotion of independence

We promote a wide range of activities which support the development of “life skills”, promote independence and are tailored to the individual’s aspirations.

The service has a small car for outings which can also be used for attending appointments. The Group has a mini bus which is used to take Service Users from Five-Ways on day trips etc.

All domestic chores and cooking is undertaken by the staff, however residents are encouraged to participate as this is a great opportunity to build on life skill development.


Accommodation

The service promotes a small homely environment which is tailored to each Service User’s individual needs. The Home comprises 3 single bedrooms. There is 1 bathroom which has a standard bath and shower. There is a separate toilet.

The bedrooms are equipped and decorated to a high standard, however, Service Users are encouraged to personalise their rooms and the home in general.

As expected, all rooms meet current fire standards and regulations. All bedrooms, corridors and communal rooms are equipped with smoke alarms. Corridor doors have automatic fire doors fitted. Emergency lighting systems are in place and fire drills are carried out on a planned regular basis.

The Home has a communal lounge with separate dining area. The lounge is equipped with a television and the Service Users enjoy Sky TV and a DVD player.

There is a large, well-established garden for the Service Users to enjoy during fine weather. The home is within a quiet residential estate close to the coastal town of New Milton.
